Housing Stabilization Services (HSS)
At Nurture Nest Services, we understand that navigating the housing landscape can be challenging. That’s why we offer personalized support to help individuals find and maintain stable housing. Through our comprehensive approach, we assist clients in overcoming various obstacles, such as securing employment, applying for benefits, and locating suitable housing. Our Housing Stabilization Services (HSS) Medical Assistance Program is designed to provide the essential support needed for each client’s unique journey toward stability and independence.
Housing Transition Services
At Nurture Nest Services, we assist clients in planning for, finding, and moving into the home of their choice.
● Creating a personalized housing transition plan.
● Assisting with benefit applications to help clients afford housing and determine their eligibility for various programs.
● Supporting clients through the housing search, application process, screenings, and assessments.
● Providing transportation to housing-related appointments.
● Offering guidance to help clients fully understand housing options and requirements.
● Securing funding for deposits, coordinating moves, and ensuring safe and stable living arrangements.
Housing Sustaining Services
At Nurture Nest Services, we assist clients in planning for, finding, and moving into the home of their choice, while also providing ongoing support to help them maintain their housing.
● Developing and regularly updating housing support, crisis, and safety plans.
● Addressing behaviors that could put housing at risk and providing preventive guidance.
● Educating clients on the roles, rights, and responsibilities of tenants and property managers.
● Offering transportation for housing-related matters.
● Assisting with applications for benefits to help clients retain their housing.
● Helping clients understand and maintain or increase their income and benefits.
● Building natural housing support systems and connecting clients to community resources.
● Collaborating with property managers or landlords to promote long-term housing retention.
● Coordinating assistive technology and adaptive accommodations to support safe living environments.

Housing Consultation Services
Nurture Nest Services offers Housing-Focused Person-Centered Plans to support individuals without case management services in finding a provider for housing transition and sustaining services. These services include:
● Assisting with obtaining the necessary documentation for Housing Stabilization Services eligibility.
● Developing a Housing-Focused Person-Centered Plan based on assessment results.
● Identifying the individual’s strengths, needs, and housing preferences, including cultural considerations.
● Supporting informed decision-making in choosing housing transition or sustaining service providers.
● Providing resources for non-housing related goals identified during the person-centered planning process.
● Coordinating with other service providers involved in the individual’s care.
● Helping clients understand their privacy rights and appeal processes.
● Annually updating the person-centered plan related to housing.
